COUNTRY: enter your country code. For this, ask your National coordinator, if present, or the Central Registry
CENTRE: enter your centre code. For this, ask your National coordinator, if present, or the Central Registry
COORDINATOR e-mail: if you are a Reporting physician you should enter your National Coordinator's email address. If you are a National coordinator you should enter the e-mail of the Central Registry: eurap@istituto-besta.it
Reporting physician: enter your name
Local e-mail: enter your e-mail address
NOTE: you can change the "Default Values" at any moment by clicking on "Set Default Values" in the Menu "UTILITIES"
